Arrive at your appointment with your paperwork completed and physical therapy prescription in hand We will copy your insurance card. You will then be seen for your initial evaluation. What will be discussed during my first visit? Your medical history Current problems/complaints Pain Intensity, what aggravates and eases these problems. How your daily life is being impacted by these problems. Your goals and wishes for your physical therapy sessions. Medications, tests, and procedures related to your health. The therapist will then perform an evaluation which includes some of the following: Palpation - touching around the area of pain looking for swelling or tenderness. Range of Motion - the therapist will move the joints to check fro quality of movement. Muscle Testing - checking for strength and quality of muscle contraction. Neurological Screening - is used to check the nerves communicate with muscle properly. Posture Assessments - position of joints relative to there ideal and current positions. A plan is then formulated using this information, input from the therapist, and input from you. The plan includes how many times you will see the therapist, home programs, patient education and goals. What do I need to bring with me? Make sure you bring your physical therapy prescription (received from your medical provider) and your payment information. If payment is being covered by insurance please bring in your insurance card. If being covered by Workers’ Compensation, bring your claim number, M1 form, and your case managers information. If being covered by auto insurance or an attorney lien, make sure you bring this information. If mailed paperwork please bring it in filled out or if you have not received paperwork we will have you fill it out at the time of your appt. How should I dress? Loose fitting clothing should be worn to expose areas that need evaluation or treatment. How long will each treatment last? Treatment sessions typically last 60 minutes per visit. How many visits will I need? This is a highly variable number The evaluation performed on the first visit will help determine this number. If more than a month is needed a new evaluation will be performed at the start of each new month.
