HOW DO I PAY

Our patients pay in combination with insurance and payment portions they are personally responsible for as defined by the contract you carry with your insurance provider. Our fees are available to you anytime and we would be happy to discuss our professional fees with you.

PAYMENT

Payment is due at the time of service for ALL PATIENTS

You may be responsible for:

Deductibles & non –covered expenses, Coinsurance as defined by your insurance plan, Co-payment as defined by your insurance plan, When a secondary or supplemental insurance is not available to you, you are responsible for payment at the time of service. Pre-authorized charges to a credit card can be arranged for treatment of minors who are not accompanied by an adult.

CASH / CHECKS / Visa and MasterCard / DEBIT CARDS

INSURANCE

Insurance coverage is not a guarantee of payment. YOU are ultimately responsible for the timely payment of your account. The contract you have obtained is between you and your insurance provider. We are not a party to this contract; however we will file clams as a courtesy to our clients.

As a courtesy we will make every effort to:

• Verify your benefits and eligibility for therapy
• Inform you as to our status whether we are a provider with your insurance company or direct you to your customer service representative
• When applicable to your policy, obtain pre-certification once services are rendered and limitations are met.
• It is absolutely necessary that you be aware of your policy limitations. It is your responsibility to have knowledge as to what is and what is not covered under your insurance policy. We will help in every way to direct you who to contact, but we will NOT become involved in disputes between you and your insurance company. Upon request, with written authorization, we will provide copies of records and supply the facts.
• All insurance claims are filed on a weekly basis.

Denial for insurance payment occurs when:

• Change in insurance status is not communicated to our office
• Lack of response to a third party liability letter

Therefore; when insurance fails to pay:

• Your bill will become your responsibility for immediate payment. If your insurance should fail to pay after 60 days of submission from our office, you will have 15 days to pay the balance without a late fee being incurred.
• Late Fees: 1.5% monthly for late payments after 30 days from the date of service
